It's actually fairly easy to get a waiver or exemption. Particularly in states that opted out of the healthcare market.

I will be paying about a $400 penalty for last year because my income is like a roller coaster resulting in over-withholding. My refund will get a $400 haircut from 5 penalty months. I was only able to get 7 months exempted.
