Does anyone have info or recommendations? Something discreet that can be activated in emergency?

Urban or something for the boonies?

There's apps for cellphones but there's flaws in that you'd have to have your location on - which is hellishly hard on your battery, and you'd have to be in range of the towers.

A satellite device would definitely give you global service but they're not cheap. The newer ones allow for text communication via satellite, which is very handy.
